October 1, 2019 through October 31, 2019

Now is the time to enroll in legal insurance, additional life insurance, short term disability insurance and discounted Life Flight Network membership for coverage beginning January 1, 2020.

The handbook includes detailed plan information and enrollment forms. Please note; all enrollment forms will need to be printed, signed and returned to SEIU Local 503 Membership Advantages for processing. The deadline to submit enrollment forms is October 31, 2019. Forms can be mailed to SEIU Local 503, PO Box 12159, Salem OR 97309, emailed to membershipadvantages@seiu503.org, or faxed to 503-581-1664.

Life Insurance

In addition to your $2,500 FREE life insurance you can purchase voluntary life insurance – if you work at least 40 hours per month in your SEIU Local 503 represented position

• You are guaranteed up to $50,000 of term life insurance.
• If you are currently enrolled, you are guaranteed up to $100,000 of term life insurance – this option is available during this open enrollment only.
• Your spouse/partner is guaranteed up to $20,000 of term life insurance.
• Your children are guaranteed up to $10,000 of term life insurance.
• You and your spouse/partner can apply for up to $20,000 of term life insurance. Approval through satisfactory Evidence of Insurability is required.

Short Term Disability Insurance

• You can purchase sort term disability insurance – if you work at least 80 hours per month in your SEIU Local 503 represented position

ARAG Legal Insurance

You can purchase the UltimateAdvisor Legal Insurance from ARAG. You will have access to a nationwide network of more than 14,000 attorneys who provide legal counsel, document review and representation.

Legal plan changes effective 1/1/2020:

• Addition of Social Security, Medical and Veterans benefits.
• Removal of felony coverage for juvenile court.
• Addition of coverage for dependent children with a disability, who are age 26 or over, if you or your spouse/partner are their court-appointed legal guardian – this option is available during this open enrollment only.
