Membership Advantages New Member Enrollment

As a new member, you can purchase life, short-term disability, DUO accident or legal insurance. You must submit your enrollment forms within 90 days of new membership.

NEW DUO ACCIDENT INSURANCE

An accident can be disruptive to daily life, both physically and financially. Whether the member, their spouse, or their covered dependents suffer an injury from something like falling off a bike, accident coverage will ensure members and their family are prepared for the unexpected. USAble DUO Accident Plan pays a cash benefit on top of any medical insurance that can be used for anything you may choose.

You must work at least 40 hours per month to purchase DUO Accident insurance.

LIFE INSURANCE

In addition to your free $2,500 member life insurance, you can enroll in supplemental life. As a new member, you are guaranteed enrollment in life insurance during this open enrollment; this is an exclusive opportunity for SEIU Local 503 members only!

Coverage is guaranteed as a new member and rates are based on age and coverage amount.

  • Members are guaranteed up to $100,000 with no health questions asked. Coverage up to $200,000 with approval through medical underwriting.
  • Spouse/partners are guaranteed up to $40,000 with no health questions asked. Coverage up to $200,000 with approval through medical underwriting.
  • Children are guaranteed up to $10,000 with no health questions asked.

You must work at least 40 hours per month to purchase supplemental life insurance. You must be enrolled in supplemental life to purchase spouse/partner or child life insurance.

NEW SHORT TERM INSURANCE

The new short term disability insurance plan pays for up to 100 weeks of benefits after a 30-day elimination period. Benefits are reduced while you are receiving Paid Leave Oregon benefits.

New members are guaranteed enrollment in the new short term disability insurance during this open enrollment.

You must work at least 40 hours per month to purchase short-term disability insurance.

LEGAL INSURANCE

As a new member, you can purchase ARAG Ultimate Legal insurance during this open enrollment period. Legal insurance provides coverage for wills, family law, traffic tickets, bankruptcy, landlord issues and more. Most issues are covered at 100% and a free identify theft insurance is bundled into the plan!

FREQUENTLY ASKED QUESTIONS

When can I sign up for voluntary life insurance, short-term disability, DUO accident and/or legal insurance?

You may enroll in each of the four insurance types within 90 days of new membership or during an annual open enrollment period in October. If submitting enrollment forms during an annual open enrollment period, the forms must be received by Membership Advantages no later than October 31.

Where can I access enrollment forms?

Enrollment forms and online enrollment links can be located on our website: seiu503.org. Members can also contact Membership Advantages to request the forms. A representative will send forms by mail, email, or fillable online enrollment link depending on your preference.

What is DUO accident insurance?

DUO accident insurance is intended to enhance and supplement your current medical coverage. DUO accident insurance offers financial benefits for covered injuries, treatments, services, hospital admission, functional losses, accidental death, and more.

How do I cancel or make changes to my current life or legal?

You may submit a cancellation request during an annual open enrollment period. All cancellation requests must be submitted in writing and will take effect January 1st of the following year. You can change enrollment outside of open enrollment with a qualifying event. To find out if your event qualifies, contact the Membership Advantages Department by email or by calling us directly at 503.772.6569.

How do I obtain a copy of my life insurance policy?

You may request a proof of coverage letter at any time by email, or by phone: 503-772-6569.

Does my life insurance policy have cash value?

No, the life insurance is a term life insurance policy and will only be paid out to the beneficiary on file at the time of death.

How can I update the beneficiary for my life insurance policy?

You can designate or update your beneficiary at any time by completing a Beneficiary Designation Form. The Beneficiary Designation Form and online link to designate your beneficiary can be found on our website: seiu503.org.
