A consistent record of abuse and neglect

There were 313 substantiated complaints of abuse, neglect, and licensing violations at Mosaic facilities between 2019 and 2024.

Willamette Springs was cited for more than 150 abuse, neglect, and licensing violations since it opened in 2016. In the past 2 years alone, Willamette Springs has been cited for more than 50 instances of abuse.
Boone Ridge was cited for more than 70 violations during the same time period, with more than half of those for physical or verbal abuse.
Mountain Park was cited for more than 55 violations between 2019 and 2024. This includes but is not limited to 15 physical or verbal abuse violations and 19 medication error violations.
Pacific Grove had more than 75 violations between 2019-2024, including 15 failing to report and 15 medication errors for residents.

Violation fines eat away at resident care budget

Between 2019 and 2024, Mosaic operations in Oregon has paid more than $153,000 in violation fines, mostly related to abuse.

Mosaic chooses profits over people

Mosaic reports $15.7 million in revenue, but entry-level pay for front-line caregivers is $15.75/hour, while Mosaic’s CEO Doug Sproul was paid $320,000/year as far back as 2020 (most recent information available)

“Within months, Michelle developed unexplained bruises, lost nearly 60 pounds, and suffered a brain hemorrhage after a bad fall in August 2023. She’d been given a sedative before bed and then left alone, in violation of state rules, inspectors later determined. Staff wasn’t helping her eat, contrary to doctor’s orders. Michelle died last March—an end hastened, her family believes, by her treatment at Willamette Springs.”

“Due to short staffing residents [at Boone Ridge] have missed multiple doses of seizure medication, residents have not received proper wound treatment which caused further harm, and multiple physical altercations.”

“[Mountain Park Care Staff] fail[ed] to provide the Heimlich after [resident] choked and [resident] died. … fail[ed] to provide basic care and services necessary, resulting in [resident] having a hematoma and scratch to the side of their forehead, creating a risk of serious harm.”

Community supporters

Workers at Mosaic Heron Pointe recently organized a union to try to address serious issues with staffing, supplies, and resident safety at their facility. Mosaic has so far refused to bargain with these workers. Workers are currently demanding Hoyer lifts in the facility due to safety hazards (workers are mandated to conduct 3-person lifts by themselves, endangering workers and residents). Take action today to stand with these workers in their fight for quality care for Heron Pointe residents.
